I definitely agree, also. We need this type of leadership to move forward and make things right again. Nice article.

It is interesting to know that Align just recently became profitable; I was under a very different impression but hadn't looked into their details.

I was planning on their June 12th Cert. I course but canceled it the moment I heard about their new quota system. I was very angry that I'd signed up just the week prior to the announcement but was told nothing of it. I fully support our professional organizations that are telling Align Technologies that they are out-of-line in attempting to impose so-called "proficiency" requirements, which is not the place of a corporation, but the rightful territory for professional organizations.

It will be interesting indeed to see what happens with ORMCO's suit.

I could not agree with you more. Leadership at the national level is severely lacking. When the American Board of Orthodontics made the decision to have universal board certification through the "buy certification" program and automatic certification by merely graduating from an orthodontic program it started down a slippery slope that has yet to reach bottom. The "dumbing down" effect has not seperated the profession from the general dentist it has merged us. The lack of any kind of standard of care the anything is OK standard has lowered the bar in our profession. Once you graduate from either dental school or any graduate program it seems the majority leave there intelligence and integrity on the stage with there graduation gown. We have abdicated diagnostic decisions to sales people. If the general dentist sees nothing but poor occlusion, severe bialveolar protrusions, lifetime retention, no functional basis and continual relapse coming back to him from his local specialists why wouldn't he fall prey to the ever present sales staff who tells him he can be doing the same thing! Before our specialty can seperate ourselves from the general dentist it needs to seperate ourselves from ourselves.

I've prepared an article that will be published in the upcoming issue of OrthoTribune that will address this issue. There are plenty of general answers -- more education, more experience, etc., etc.. There are plenty of signals people look for like fees (specialist are expected to charge higher fees), the use of new technologies that are meaningful to the patient, and the general appearance of the practice. But your question is one of the core issues the profession must address -- what is the difference or has technology made GPs as effective as orthodontic specialists? The profession needs to spend some quality time on this issue and define a clear position. My meager understanding would lead me to believe that it is simple to start most cases, but a specialist is revealed in the finishing...

How would you go about explaining to a patient the virtues of going to a specialist for braces rather than a GP?

Mr. Blake, that seems to be an excellent way for doctors to help clients get financed in this tight market. I think the doctor that has the smoothest assistance program will do well... I would also invite acceptx.com to take a look at having a presence on cosmetic surgeon directory.
